Thus far when processing trades, I have taken budget room and cash to be synonymous. I'm holding a vote to determine if perhaps I should hold a more strict definition of "cash," meaning that if a team has budget room but has agreed to send a specific quantity of cash that they do not have, then the trade will be voided.

Just my 2 cents, but in the real world MLB teams have access to credit lines that would allow them to make "cash" deals without requiring the money to be sitting in a deposit account somewhere. OOTP's not the real world and from a game standpoint requiring cash on hand might be a better approach, but the more realistic way to handle it is through the budget process. Also, I'm not sure in this league, but in my other leagues some owners prevent GM's from accumulating cash (I'm looking at you David Glass, grrrr). Requiring cash on hand basically precludes those teams from making cash deals, which is just another disadvantage for GM's dealing with difficult owners.

Of course the big markets want budget space and not cash on hand to determine things... Making cash cash is the fairest system for all.

I don't really see it as a big market vs. small market issue. When I was in Miami I turned a profit almost every year, but rarely had cash because the owner took it. I don't think there's actually much practical difference between cash and budget space. If I don't have cash but want to give you money, I can pay the salary of one of your players, which achieves the same thing. Mostly I'm just generally against things that add another restriction that isn't enforced by the game, because invariably someone will overlook it.
