Should we put a cap on International Free Agency? Right now there are some penalties for going over the $5 million threshold, but there isn't a cap.

The current rules really favor big market teams with large budgets. Every two years you can go crazy and sign all kinds of players, then you sit out a year, and do it again. I spent $23M+ on IAFAs last season and over the years have picked up several eventual top-100 prospects this way. Most smaller and mid-market teams can't do this.
Caps create some problems - signing guys becomes more random - but it does give smaller budget teams a more equal shot at the top IAFA guys. There's not a great answer given how OOTP handles IAFAs, but I think the cap system is better for overall league competitiveness and balances the playing field more than the current system.

I've mentioned this before, but I'm not convinced OOTP appropriately manages this setting if multiple teams offer the max on a hard cap. For example, if multiple teams offer $5MM, the export that is pulled first may win the player.

Not sure I have an answer, but I did some testing on this in the base game. I ran several sims to IAFA in game, jumped into commish mode, and had multiple teams make the same $5M offer to a guy asking for the $5M cap amount. In every case, the prospect chose the team that make the last offer. Ex, in one sim I had AZ, CHC, HOU, MIN, OAK, SEA, TX, WAS make identical offers in that order, the prospect chose WAS. Then I reversed it for the same prospect, he chose AZ. I noticed during the neg phase, prospects would on occasion say how much they liked the org. That didn't have anything to do with their choice of teams in my sims. Not sure how this works in an online league, but in the base game at least the AI logic in deciding where an IAFA prospect signs with multiple, identical offers isn't random. It seems to just be based on who made the last identical offer.

I am in a league in which each team starts with a $5 budget to sign international free agents, but you can trade all, or some, of that budget for draft picks, prospects, or whatever… basically that budget is tradable. I thinks that works pretty good.
